It's important to get feedback from your customers about your products and services. Many customers don't give you feedback until something goes wrong. Use a comment card like this to encourage your customers to let you know what works, and what doesn't work.
Remember, when an unhappy customer leaves your premises they'll tell 20 people, yet a happy customer will only tell 2. Couple this comment card with a free voucher, or prize, to encourage participation.
